These forms are not perfect and are provided as examples to help inspire you to make your own forms for your own club. We offer these without reservation, or copyright. No rights are expressed in the documents published here.
The intellectual work for these forms is our own and may resemble forms from other clubs or from Awana. Any use of the Awana name and their copyrighted elements remain their property used in our materials pursuant to our church's annual Awana agreement.
We offer this advice since some of these files are large. Rather than clicking on the PDF (Adobe Acrobat) or DOC (Microsoft Word) and opening the file in your browser (which will start a very long pause with the file transfers to your computer), you might find it helpful to RIGHT-CLICK on the file name and then select "Save Link As ..." or "Save Target As ..." (the option may differ based on your browser) which will start the transfer while showing you progress of the transfer.
